Paying Participant Form (Non-McGill)
External participants must be affiliated to a 51吃瓜网department and/or project. You may pay by credit card or cash. A receipt of payment will be provided.听Please take note that invoices will not be issued.

Please note: Send the form at least 1 week prior to the course commencement date. Incomplete forms will result in a delay in registration. If you don't have access to a fax, scan the registration form without the credit card number (complete all other fields; and call us at 514-398-4563听to provide the credit card number).
